ESA Basic Level Membership Form

This membership is for five family members, whether they live in the same household or not. One person may be called a generic "Caregiver" to provide the flexibility to cover any babysitter or relative. Memberships is valid for three weeks until payment is received and then it will be extended for the year of membership, according to the date application was made. Membership cards will be mailed within five to 10 days of payment. Express mail is not available. You may request to pick up at the museum and you will be emailed when the membership cards are available.
