07-Secondary Form for Basic & Premier Memberships

Our memberships require the relationship and child birth date information that is not collectible on the membership form you already filled out. Thank you for your patience in filling out a second form. Membership cards will be mailed within five to 10 days. Memberships are non-refundable. Express mail is not available. You may request to pick up at the museum and you will be emailed when the membership cards are available.
